It has a similar presentation to ovarian cancer. It has a poor prognosis with survival ranging from 12-18 months. Metastasis involves mainly transperitoneally, but lymphatics and hematological metastases have been also reported. It is a diagnosis of exclusion made after pathological report (1) Here is a case of a 40-year old female who presented with crampy abdominal pain that progressed into abdominal distention and was diagnosed with Serous Surface carcinoma stage III with high consideration of peritoneum as primary. The patient was advised to have chemotherapy but was not able to comply due to financial problems. Two subsequent admissions were due to pleural effusion and eventually lead to the patients` demise ER -
