Such attack can be precipitated by strenuous exercise, diet with high carbohydrate and sodium content; stress, cold temperature and some agents given during anesthesia. Presented is a case of a 38 year old make with history of HKPP 4 years ago, underwent emergency appendectomy under regional anesthesia and developed loss of consciousness, respiratory insuficiency, hypotension, tachycardia, hypercarbia, and hypokalemia. These intraoperative events strongly suggest the possibility of HKPP attack. Under those circumatnances, necesary resuscitative and medical management were done, preventing patient from further harm and sequelae that can be brought about by such attack ER -
