Physical findings showed pallor, multiple cervical and inguinal lymphadenopathies, hepatomegaly, splenomegaly, petechiae on the anterior chest extending to the neck and groin areas and atrophy of lower extremities. Initial blood work-up revealed anemia, thrombocytopenia, leukopenia, neutropenia and lymphocytosis. Patient also had a past history of fall as well as exposure to an individual with active pulmonary tuberculosis. The patient was initially managed as a case of primary pulmonary tuberculosis to consider Pott`s Disease. Further workup revealed the presence of other disease entities. Flow cytometry of bone marrow aspirate showed immunologic findings consistent with a blast population showing both myeloid and B-lymphoid antigen expression, thus giving a diagnosis of Acute Biphenotypic Leukemia ER -
