N2  - ABSTRACT: The management options in a neonate with bilateral midline vocal fold paralysis was presented.  The role of tracheotomy versus watchful waiting and the ethical concerns in the management of the case was discussed.  Watchful waiting provided us the opportu to observe the natural course of the condition.  This led us to decide that prudence would dictate that watcful waiting is not a management option in a case of neonatal bilateral midline vocal fold paralysis.  In cases where the patient or guardian refuses treatment, respect for patient choice should be para,ount even if leads to poorer outcome
