520 _aABSTRACT: Heterotopic pregnancy is a rare event in natural conception cycles. A high index of suspicion can help in timely diagnosis and appropriate intervention. We report a case of heterotopic pregnancy in a 3 year-old Gravida 4 Para 3 (3003) presented with hypogastric pain and hemoperitoneum from ruptured tubal pregnancy with live intrauterine gestation at 7 weeks of amenorrhea, diagnosed on ultrasound examination who underwent exploratory laparotomy, evacuation of hemoperitoneum and left salpingectomy Heterotopic pregnancy can occur in the absence of any predisposing risk factors. A high of suspicion is essential in the low risk symptomatic patient with abdominal or pelvic pain in which ultrasound findings are consistent with intrauterine gestation sac while free fluid is noted in the pelvis with or without an adnexal mass. Only with an early diagnosis and treatment the intrauterine pregnancies will reach viability with a great chance of a favorable obstetric outcome.
