520 _aABSTRACT: This is a case of 61-year-old male with multinodular nontoxic goiter who underwent total thyroidectomy under general anesthesia – laryngeal mask airway after multiple failed intubation attempts. The patient had a Cormack-Lehane grade of 4 under direct visualization. Despite changing operators, blade size and endotracheal tube sizes, intubation was not successful. A laryngeal mask airway device size 4 was then successfully inserted, with equal bilateral breath sound and successive end tidal waveform. There was no airway leakage, no desaturation nor dislodgment of the laryngeal mask airway device noted throughout the seven-hour thyroidectomy.
