520 _aABSTRACT: Weil`s disease is a severe form of leptospirosis, which occurs in 5-10% of cases. The presence of this disease in a 58 year-old hypertensive male previously diagnosed with chronic kidney disease and artrial fibrillation, who presents with acute renal failure. jaundice, fever and muscle pain, is presented. Appropriate antibiotic therapy was given and hemodialysis was instituted. During the hospital stay, patient`s liver function tests revealed abnormal results and presented with crampy, epigastric pain. Magnetic resonance imaging (MRI) of the abdomen and magnetic resonance cholangiopancreatography (MRCP) confirmed a diagnosis of Caroli`s disease, which is a rare congenital disorder of the biliary system. Patient is currently on hemodialysis twice a week and on constant follow-up for possible complication of Caroli`s disease. Pathophysiologic interaction of both disease affects the overall condition of the patient, particularly on the liver function of the patient.
