520 _aABSTRACT: We report a rare case of olfactory neuroblastoma in a 23 year old female who presented with 3 month history of recurrent nasal congestion associated with intermittent headache, unilateral epistaxis and proptosis of left eye. Cranial MRI with contrast showed a large enhancing sinonasal and anterior skull base mass measuring 6.9x6.3x6.1 cm. Enlarged lymph nodes, right high jugular region and both lateral retropharyngeal spaces. Nasal endoscopy with biopsy revealed fibrocollagenous tissue composed of round cells arranged in sheets, cords and clusters some are separated by thin fibrous tissue. The cells have mild pleomorphism, granular chromatin pattern and scant cytoplasm. With areas of necrosis, congestion and hemorrhage. The findings are consistent with olfactory neuroblastoma. Immunohistochemical staining for Synaptophysin revealed positive, diffuse and strong cytoplasmic expression in neoplastic cells. Chromogranin revealed negative expression in neoplastic cells.
