Summary: Abstract: Heteropagus twin, also known as parasitic twin, is an extremely rare type of monochorionic twinning classified as external asymmetrical in which a partially formed defective twin is attached to a normal twin. Prenatal diagnosis requires careful evaluation and use of multiple modalities. Multidisciplinary approach involving different specialists must be included in the term to come up with a definitive plan of delivery. Additional findings such as an adnexal mass should be accurately evaluated to identify the need of a immediate surgical intervention. The objective of this report is to present a case of a 24 year old primigravid who delivered a parasiticus twins via cesarean section. Early diagnosis at 25 weeks age of gestation enabled an extensive antepartum planning. The delivery was followed with left cystectomy with frozen section with findings of seromucinous tumor with focal stratification and atypia representing less than 10% of the sampled material. This was followed with left oophorectomy. Final histopathologic result still benign. For the twins, successful from Twin A was done by another institution with an outcome of a craniopagus parasitic twins with components consistent with an immature teratoma. Close monitoring and follow-up was advised. Once stable, the surviving twin will undergo facial reconstruction.
