ABSTRACT: This is a case of a 31 year old male completely asymptomatic, with an incidental finding of posterior mediastinal mass on routine chest radiograph. The objectives are to present a rare case of an asymptomatic patient with an incidental finding of a posterior mediatinal mass. To discuss the diagnostic approach in evaluating patient with a posterior mediastinal mass. The initial consideration was neurogenic tumor. Patient underwent CT-guided biopsy of the mass twice but both but specimen were inadequate. Patient underwent right open thoracotomy with incision biopsy. Hematoxylin and Eosin Stain revealed Epithelioid Hemangioendothelioma. The dilemma is to remove a vascular mass in an asymptomatic patient or let the patient live a normal life and not to undergo a very risky procedure. The decision in this case is to not to undergo tumor resection at the moment and monitor the progression of the mass by CT - Angiography.
