Summary: Abstract: Dilated cardiomyopathy, a myocardial disorder characterized by a dilated left ventricular (LV)chamber and systolic dysfunction that commonly results in congestive heart failure (CHF). This is a case of V.G., a 3 year male who presented with easy fatigability and irritability for a month and was admitted due to bilateral nonpitting edema, undocumented low grade fever, productive distressing cough, decrease appetite and activity. Congestive heart failure (CHF) is the inability of the heart to meet the metabolic demands of the body. The population at greatest risk of heart failure are children with dilated cardiomyopathy (DCM). Dilated cardiomyopathy is myocardial dysfunction causing heart failure in which ventricular dilation and systolic dysfunction predominate. This paper aims to present a case of congestive heart failure in a 3 year old male, emphasize the importance of complete history taking and physical examination in the diagnosis of a cardiac pathology, recognize the sign and symptoms of impending heart failure, discuss the prognosis, current treatment options of children with heart failure and discuss the role of the pediatrician in helping the parents/family understand the outcome of the disease.
