Summary: ABSTRACT: This is a case of a 57 year old female, diabetic for 3 years, with a strong family history of colon and gastric cancer, who sought consult with a private physician due to on and off one sided headache accompanied with occasional dizzines. No other symptoms were noted such as vomiting, fever, cough, weight loss, loss of appetite, abdominal pains. She was given Ibuprofen (Advil) 200 mg/ tab which relieved her symptoms and was advised to have a screening colonoscopy done because of her family history. Colonoscopy report was: Polyp, transverse and sigmoid colon; Chronic appendicitis, rule out malignancy. Biopsy result was hyperplasia of polyp at sigmoid, tubular adenoma at the transverse colon while periappendix/cecum biopsy revealed a chronic granulomatous with Langhan`s type multinucleated giant cells suggestive of a tuberculous infection. Laparoscopic appendectomy was done after which, anti TB regimen as started.
