Abstract: This study aims to design a mental health application in terms of the factors that triggers the symptoms of anxiety that individuals may experience due to certain conditions. The identified factors that trigger anxiety was used to design a mental health mobile application suited for the people who are experiencing symptoms of anxiety. This study is conducted among the 12th grade of Senior High School Students of Far Eastern University - Nicanor Reyes Medical Foundation. The main objective of this study are to describe the common problems or factors that makes teenagers anxious and to determine how a mental health application can aid with experience anxiety. Data was gathered through distribution of survey-questionnaire, and through the interview conducted via zoom. The data were analyzed with the use of tables and graphical representations for possible recommendations for the beneficiaries, especially the students. This study used a mixed-method research design in collecting data from the informants. The results concluded that the common problems that affects the student's mental health are school workloads and isolation. Also, their reasons for not seeking help are fear of being judged by others, feeling of being a nuisance, and feeling of being disturbance to others. Lastly, majority of grade 12 students favored mental health applications as they believe that it reduces signs of anxiety. Hence, a mental health application design was developed considering these factors.
