Abstract: A 12-year old male presented with sudden, excruciating, intolerable, left sided headache. Plain cranial CT scan revealed left frontoparietal bleed with intraventricular extension. Thus, emergency craniotomy with evacuation of intracerebral hemorrhage and ventriculostomy were done. The identification of the etiology of the stroke in the young may be easy but it is more often, very difficult and frustrating. Stroke has abrupt or rapid onset implying its vascular pathogenesis, It affects at least 1 in 4000 newborns and approximately 2000 older children per year. Hence, a multidisciplinary approach is essential in both the diagnosis and proper treatment.
