Summary, etc. Abstract: Listeria monocytogenes is an uncommon etiologic agent causing acute bacterial meningitis worldwide. In this case, we have a 42 year old male who presented with fever, changes in sensorium, nuchal rigidity, and cranial nerve palsies. He was treated as a case of tuberculous meningitis vs. bacterial meningitis and was given a combination of anti-Koch's treatment and Ampicillin-Sulbactam. Upon admission, CD4 levels were also taken, revealing low CD4 count, making him an immunocompromised host. CSF analysis was done and post-admission, revealed L. monocytogenes as the etiologic agent. The dilemma existed regarding the proper antimicrobial treatment to be administered. This case report aims to discuss the diagnostic approach and management of patients with Listeria meningitis.
