Abstract: Xanthogranulomatous Pyelonephritis is a rare disease, characterized by chronic suppurative renal inflammation that leads to continuous renal parenchymal destruction and worse, resulting in a non-functioning kidney. It is suspected in patients with major anatomical abnormalities and history of recurrent acute pyelonephritis. This disease is predominantly seen in women than in men, essentially in all age group, but most of them are seen in middle aged to elderly patients. Xanthogranulomatous Pyelonephritis results from the formation of pus or granulomas, associated with long standing sequelae of repeated urinary tract obstruction and infection. Studies reported that there is an increase incidence in those patients with Diabetes Mellitus and Transplanted Kidney. There are two forms associated with this disease, the diffuse and the focal form, both of which are seen on imaging studies. The diffuse form is the more common and accounts for 85 - 90% of the disease while the focal or localized form accounts for 10-15%. The radiographic feature of the disease is usually specific. Plain radiograph and ultrasound examination can help in the diagnosis but Computed Tomography scan is believed to be the most helpful diagnostic imaging. However, there is no conservative or medical approarch recommended in the treatment of xanthogranulomatous pyelonephritis. Some studies say that medical approach is infrequently advised because it cannot suffice for treatment. But, in all cases,antibiotics are still used. Surgical nephrectomy or in rare cases, partial nephrectomy is the standard treatment |
