Summary, etc. ABSTRACT: This is a case of a 70-year old female with a history of a prolapsed uterus. After appropriate medical and surgical consultations, she underwent an unremarkable vaginal hysterctomy, anterior and posterior repair under subarachnoid block. However, five hours after the surgery, while at the ward, she suddenly became unresponsive. Her oxygen saturations dropped and she went into asystole. Cardiopulmonary resuscitation was done and return of spontaneous circulation was achieved after one ampule of epinephrine. With a primary consideration of Pulmonary embolism, anticoagulant therapy was started. She was stable enough to be extubated after two days and hence was able to verbalize that she had a " pins and needles" on her legs. A quick Neurologic exam revealed decreased sensorium and motor weakness on both her lower extremities. Diagnostic imaging revealed an intraspinal hematoma. She was deemed high risk by medical evaluation. After a multidisciplinary conference, the family decided to bring the patient home against medical advice. Keywords: spinal hematoma, adverse effect, epidural hematoma, spinal anesthesia, anticoagulant.
