"The Population Health Analytics textbook is a playbook to design, build, and implement a Population Health Management strategy. The targeted audience of this book are health care leaders and their clinical, analytic, technology, and operations partners who have 1) seen how data in other industries is used to improve performance, drive profitable growth, and solve complex industry challenges; 2) committed to implementing a data driven Population Health strategy in their healthcare organization and in their community; 3) have gathered the clinical, analytic, technology, and business operations resources; and 4) share in the Triple Aim goal of improving the patient experience of care, improving the health of populations, and reducing the per capita cost of health care through the practical application of analytics and data driven decision-making.
