Summary, etc. Abstract: This is a case of C.E, a 5 month old male who initially presented with respiratory infection and was subsequently to have a large anterior mediastinal mass seen in Chest CT scan and was later identified as Cystic lymphangioma in a histologic report after surgical thoracotomy. Surgical intrvention represents the treatment of choice, thus exploratory is indicated once the lesion is found to establish a tissue diagnosis and to extirpate the mass. The patient underwent thoracotomy with incomplete resection of the anterior mediastinal mass due to adherence of the mass to nearby vessels and tolerated the procedure well. Unfortunately after few weeks post-operation, repeat chest xray was done and showed recurrence of the said mass. Patient then underwent intralesionalbleomycin injection. This paper aims to discuss the different etiologies of anterior mediastinal mass and their treatment options for infant patients. Mediastinal masses are relatively rare and encompass a wide variety of diseases from the purely benign to the extremely malignant. Mediastinal Mass is rare in children, and usually 50% of pediatric mediastinal masses are malignant, therefore needs to be investigated thoroughly. It also have a diverse origins and etiologies. In order to end up in a correct diagnosis, logical and practical strategy is needed by the help of diagnostic imaging and laboratory test to be done in such cases, which can help in planning for further invasive diagnostic procedure as well as therapeutic decisions for proper management.
