Summary, etc. | Abstract: Anaphylaxis during anesthesia is a rare event which can have dramatic and unpredictable consequences. In 60-70% of cases. It is secondary to neuromuscular blocking agents (NMBAs). The worst manifestations include cardiovascular collapse, brochospasm and laryngeal edema. The overall incidence of perioperative anaphylaxis is estimated at 1 in 10,000-20,000 anesthetic procedures, whereas it is estimated at 1 in 6,500 administrations of NMBAs. Correct management of anaphylaxis during anesthesia requires a multidisciplinary approach with prompt recognition and treatment of the acute event by the attending anesthesiologist. Presented is a case of a 43 year old male who underwent Emergency Exploratory Laparotomy under general anesthesia. The patient had no history of allergies and had never undergone general anesthesia before. After the patient was induced and intubated, he was given Rocuronium for muscle relaxation. Approximately ten minutes after rocuronium administration, tachycardia, O2 desaturation and hypotension were noted. The patient was then given Ephedrine and hyrodcostisone and was shifted to Atracurium after a few more episodes of hypoxia and tachycardia upon administration of Rocuronium. After the patient was stabiilized, surgery resumed without complications. |
