ABSTRACT: Mannitol, is an osmotic diuretic that has been widely used in different clinical settings. As a decompressant agent, it is used in cases with increased intracranial pressure that has been a problem in the management of acute stroke. However, in an attempt to induce duiresis, several studies have reported that its use resulted to acute kidney injury which has been proven to contribute to increased morbidity and mortality in stroke patients. This study aims to determine the clinical predictors and incidence of mannitol-induced AKI among stroke patients admitted in FEU-NRMF Medical Center from March 2013 - August 2013. Definition of Acute kidney injury (AKI) is based on the Acute Kidney Injury Network as when the serum creatinine rises by ≥ 26μmol/L within 48 hours or rises ≤ 1.5 fold from the reference value, which is known or presumed to have occurred within one week or urine output is < 0.5ml/kg/hr for >6 consecutive hours. This is a prospective cohort study of 58 adult stroke patients. Patients were divided into two groups: those who received and did not receive mannitol. Serum creatinine levels were measured on the day of admission, day 2, 5, and 7 post ictus and urine output was monitored every 6 hours. Outcome will be based on the above definition of AKI based on the Acute Kidney Injury Network. Patients who sustained acute kidney injury (AKI) were reviewed based on the demographics and clinical settings that may strongly predict or contribute to the occurrence of AKI. |
