Summary, etc. The ovarian remnant syndrome, a complication of bilateral salpingooophorectomy, is progressively receiving more attention in the gynecological surgery literature. The syndrome is manifested by pelvic pain and a palpable pelvic mass. Abdominopelvic CT scan or sonographic findings is of little help in the diagnosis. The definitive criteria for the diagnosis are clinicalhistory of bilateral sallpinoooophorectomy and histologic documentation of an ovarian tissue. We present the case of a 42-year-old Asian female, Gravida 1 Para 1 (1001). Previous gynecologic surgeries such as left salpingooophorectomy and right oopphorocystectomy and total abdominal hysterectomy with right salpingooophorectomy were done 9 years and 9 months and 6 years and 9 months respectively. Various adhesion-producing conditions leading to retention of ovarian tissue, such as endometriosis, and pelvic inflammatory disease were present at the above mentioned procedure. She complained of constipation and pelvoabdominal mass. Radiologic evidence of an ovarian remnant was discovered. Subsequent resection via exploratory laparotomy was consistent with a mucinous adenocarcinoma with invasion to the ileum. Currently, she is on her 5th cycle of combination chemotheraphy with Paclitaxel and Carboplatin.
