Summary, etc. Obstetrical complications as hemorrhage may ensue once the placenta adhere into the myometrium and was not noted during placental delivery. The objective of this report is to present the case of a 30-year-old, Gravida 3 para 1 (1021) who had persistent vaginal bleeding post dilatation and curettage due to missed abortion at 11 weeks gestational age. Measurement of serum beta human chorionioc gonadotrophic hormone, transvaginal ultrasound and pelvic MRI with contrast were done leading to a possible retained product of conception, gestational trophoblastic neoplasia, uterine arteriovenous malformation, and placenta increta. She underwent Exploratory Laparotomy followed by Total Abdominal Hysterectomy under General Endotracheal Anesthesia. In the current generation, wide array of diagnostic work-ups can be used to make an appropriate diagnosis. And with correct management, correct management can be given. With the case, we were faced with a patient with financial constraints. But what is more strucking is that our final histopathology is different with our final diagnosis. The case was an eye opener for the service team. After encountering the case, the moral lesson of which is to correlate our diagnostic tests made with the patient. We must never forget to take into consideration the basic and the most common.
