Summary: ABSTRACT: This is a case of a 24-year-old male presenting with difficulty of breathing associated with 1 week history of undocumented fever and productive cough, initially managed as a lung abscess as seen on the chest radiograph. On further work-up, it was found in contrast-enhanced CT scan that there is presence of intralobar sequestration with concomitant abscess formation. Patient subsequently underwent thoracotomy and left lower lobectomy. Histopathology findings revealed interstitial pneumonia with caseating granulomatous inflammation with thrombus formation and necrosis. Hence, patient was started on anti-tuberculosis medications.
