ABSTRACT: This is a case of a 62 year-old, male, who is a known case of lung adenocarcinoma with brain metastasis, who presented with right lower quadrant abdominal pain described as colicky, intermittent, non-radiating, and is not associated with food intake. Initial impression at the time of admission was acute appendicitis. Whole abdominal ultrasound and whole abdominal CT-scan with contrast were done which revealed a complex mass lesion in the right lower quadrant, consider abscess and phlegmon and consider acute suppurative appendicitis with probable phlegmon or abscess formation, respectively. Patient underwent exploratory laparotomy, appendectomy and the official biopsy result showed poorly differentiated adenocarcinoma, metastatic to the appendix and liver.
