Summary: Abstract: Twins vs. singletons a higher risk for perinatal morbidity and mortality. Discordancy in twins imparts various complications post-natally which includes prematurity, low birth weight and infection. This is a case of twins born to a 31-year-old GIP2(o1o2) with an ultrasonographic finding of a multiple fetus in breech presentation with grade 2 placenta monochorionic, diamnionic and a histologic findings of monochorionic diamnionic placenta measuring 19.5x15x3cm weighing 600g.. There is a very delicate, filmy amniotic membrane, which separates the two umbilical cords. Twin A had a birth weight of 200g and Twin B had a birth weight of 1070g, Sepsis work up was done for the twins. Twin A was discharged with a diagnosis of Twin A, live, preterm, (34 weeks by Early Ballards) twin, male via Emergency Low Segment Cesarean Section due to non-reassuring heart tone pattern. APGAR score of 9 and 10, birthweight of 2000 grams, appropriate for gestational age. Prematurity. Transient Tachypnea of The newborn. While twin B stayed longer at the NICU and manifested complications brought about by discordancy and was discharged later with a diagnosis of Twin B, live, preterm, (34 by early Ballards) twin, male via Emergency Low Segment Cesarean Section due to non- reassuring heart tone pattern. APGAR score of 9 and 10 birthweight of 1070 grams, small for gestational age, symmetrical. Prematurity, Chordee. Transient Tachypnea of The newborn. In the Philippines it was noted that out of 4,269,722 outcomes there are no cases reported as discordant twins.
