Summary: Abstract: A standardized instrument is a tool used to assess a patient's status and establish a baseline that will help monitor changes in a patient's status over time. Despite the advantages of using it, physical therapists have some distance applying this tool in the practice. This study aims to know the magnitude of utilization of outcome measures used for acute stroke patients in the local clinical setting. A descriptive, cross-sectional study design that audits medical records in utilization of acute care outcome measures in stroke patients a prescribed by evidence-based guideline for evaluation recommended by the American Neurologic Physical Therapy 2018. Out of the 19 acute-care stroke outcome measures recommended by Academy of Neurologic Physical Therapy (ANTPY) 2018, only Modified Rankin Scale were utilized in the acquired medical records. It is concluded that the Modified Rankin Scale is the most utilized outcome measure for patients with acute stroke in selected tertiary hospitals.
