ABSTRACT: An uncommon, enigmatic variant of type I diabetes mellitus, known as the fulminant variant or type lb diabetes mellitus, is defined by its sudden and explosive onset leading to life-threatening metabolic abnormalities. Fulminant type I diabetes, which differs from common classical type I diabetes, is characterized by its quick fulminant progression, severe hyperglycemia, and, most importantly, the frequent absence of autoantibodies associated with the autoimmune destruction of beta cells. This variety frequently affects younger people, such as teens and young adults. It is troublesome to make an early diagnosis and manage fulminant Type I diabetes mellitus as its exact pathophysiological mechanisms and etiology arc still poorly understood. The clinical presentation, laboratory results, and potential therapeutic implications of fulminant T1DM arc all covered in detail in this presentation, which also includes an extensive overview of current information and diagnostic criteria. Further study is required to clarify its pathophysiology and support improved clinical outcomes for people affected as the medical community works to understand the complexity of this uncommon form of diabetes. OBJECTIVES: To present a case of a 30-year-old female newly diagnosed with Type 1 Diabetes Mellitus absent of diagnostic antibodies, with an initial presentation of Diabetic Ketoacidosis. 2. To present literature regarding Type 1 Diabetes Mellitus with absence of antibodies. 3. To discuss screening and diagnostic criteria of Fulminant Type I Diabetes Mellitus
