Summary: ABSTRACT: introduction: Adnexal masses pose diagnostic challenges due to varied etiologies. Prompt recognition is vital for optimal patient outcomes. The Ovarian-Adnexal Reporting and Data System (O-RADS) offers a standardized approach categorizing risk based on structured analysis of imaging features. Objective: Determine the accuracy of the O-RADS scoring system in diagnosing malignant adnexal masses among patients seen at a tertiary medical center. Methods: This is a cross-sectional study which included women with adnexal masses assessed by a two-dimensional ultrasonography with O-RADS scoring system and later underwent surgery. Sensitivity, specificity, positive and negative predictive values and likelihood ratios were computed for O-RADS with histopathology as the reference standard. Prevalence of the adnexal mass according to menopausal status and ultrasound features were the secondary outcomes. Results: Eighty-six women were enrolled in the study, with 29.07% (95% Cl: 19.28%, 38.86%) diagnosed with malignant adnexal mass. Based on O-RADS scoring, 31.4% (95% Cl: 21.39%, 41.4%) of adnexal masses were classified to be malignant. The O-RADS demonstrated 88% sensitivity (95% Cl: 81.13%, 94.87%); 91.8% specificity (95% Cl: 86.01%, 97%); 81.48% negative predictive value (95% Cl: 73.27%, 89.69%); 94.92% positive predictive value (95% Cl; 90.27%, 99.56%); 10.74 positive likelihood ratio (95% Cl: 4.58, 25.17); and 0.13 negative likelihood ratio (95% Cl: 0.05, 0.38). Overall accuracy was 90.70% (95% Cl: 82.49%, 95.9%). Conclusion: The O-RADS is reasonably proficient in detecting malignant adnexal masses.
