Anatomical variations of the cystic duct often occur and may be encountered during cholecystectomy. Knowledge of the variable anatomy of the cystic duct and cysticohepatic junction is important to avoid significant ductal injury in biliary surgery. We report here an extremely rare case of duplicated cystic duct that was associated with choledocholithiasis, and this malady was noted intra-operatively. Searches done from the Philippine Journal of Surgical Specialties and Cochrane revealed no locally published case on duplicated cystic duct making this report the first of its kind in the Philippines.
