Abstract: Morbid obesity is an important health problem with increasing incidence constantly. It is associated with various pathophysiological changes which affect the outcome of anesthesia and surgery. The anesthetic approach of morbid obesity has specific problems that should be investigated and optimized in preoperative, intraoperative and postoperative period. Presented is a case of a 44 year old woman weighing 142 kg with a height of 162 cm (BMI= 53.78) who underwent dilatation and curettage due abnormal uterine bleeding. The anesthesiologist chose to do a subarachnoid block. Technical difficulty in identifying bony landmarks in an obese patient was encountered and by using an extended spinal needle and placing the patient in a sitting position, block was done successfully.
