Summary: ABSTRACT: An autopsy done for purposes of determination of cause of death is a usual practice, but when unexpected finding arise, the post mortem examination becomes a basic learning tool.We are presenting a 41 years old male, welder by profession for 8 years, 30 pack years smoker and an alcohol drinker who came , In at our institution for evaluation of epigastric pain of 3 weeks duration. Physical examination done revealed sever epigastric pain and generalized jaundice. Work ups done (CT scan, USG of the abdomen) are consistent with liver malignancy, with FNAB findings of adenocarcinoma. Palliation was only contemplated. Patient expired on the 28th hospital day and post mortem examination was done. Autopsy findings were centered on the liver which on microscopic examination revealed evidence of combined hepatocellular carcinoma and cholangiocarcinoma, with extensive intrahepatic spread and multiple metastases.
