Summary: ABSTRACT: This is a case of E.T., 53 year old male presenting with a gunshot wound on his right lower eyelid and limitation in lateral gaze, left eye. Skull X-ray was done which revealed a metallic slug superior to the sphenoidal sinus and inferior to the left orbital bone. While CT scan showed depressed fracture, right maxillary antrum with fracture lines in the adjacent bones, soft tissue swelling, right malar orbital region and frontal region, and a high velocity projectile slug lodged in the leftward sphenoid sinus with few tiny scattered fragments. With the imaging results, the final diagnosis was Gunshot Wound; Point of Entry Lower Eyelid, Right: Point of Exit, None, Lateral Rectus Palsy, OS, Depressed Maxillary fracture, Right. The ENT-HNS service contemplated on removing the foreign body and requested CT angiogram to better visualize the proximity of the metallic foreign body to the major vessels.
