Abstract: The cause of bilious vomiting in a neonate is malrotation with midgut volvulus until proven otherwise. It occurs in 1 out of every 6000 births. It is considered a surgical emergency which should be addressed immediately. This is a case of a full term male neonate who was born to a 29-year-old G 1 P 1 (1001) mother with an unremarkable maternal history. Patient had unremarkable physical examination at birth. Post-feeding bilious vomiting started at the 9th hour of life while bloody stool appeared at the 22nd hour of life. A possible intestinal malrotation with volvulus was considered. Patient underwent Ladd Procedure, Jejunoileal Resection with Primary Anastomosis, LLeal Resection with Anastomosis. During the course of hospital stay, patient developed neonatal pneumonia and sepsis and was treated accordingly. Patient was eventually discharged improved.
