Abstract: Difficult airway may present as difficult intubation or difficult ventilation , or much worse, both. One of the scenario wherein the latter can be seen are with patients who have huge masses that impinge on the upper and lower respiratory tract. These masses has the potential to collapse on the part of the airway it is adjacent to and cause an obstruction that can be so severe as to completely obliterating the lumen. This case aims to discuss the management of airways with extraluminal compression, and to propose possible explanation for the ventilation problem encountered intraoperatively.
