Summary: Abstract: Presented is a case of J.L. a 32-year-old female with a right infra-auricular mass. This patient was initially diagnosed with Pleomorphic Adenoma of the right parotid gland after undergoing Parotidectomy with Facial Nerve preservation and frozen section. Tissue sections revealed a salivary gland harboring a malignant tumor arranged in sheets on a background of desmoplastic stroma with dense lymphocytic infiltrates. The neoplastic epithelial cells exhibit pleomorphism with hyperchromatic nuclei and some with prominent nucleoli, with hyperchromatic nuclei and some with prominent nucleoli, with a diagnosis of Lymphoepithelial Carcinoma of the Parotid gland. The patient was then lost to follow up. Patient was apparently asymptomatic after the surgical procedure, until the second trimester of her third pregnancy where she presented with a rapidly growing, moist, exophytic mass at the right infra auricular area approximately 1.5 by 0.5 cm. Wide excision of the parotid mass, right, with pectoralis major myocutaneous flap reconstruction was done ten months after recurrence of tumor. Final histopathologic report reveals a Poorly Differentiated carcinoma with a primary consideration of Lymphoepithelial Carcinoma. Immunohistochemical staining for cytokeratin, CD3, CD20 and CD45 was requested to confirm diagnosis.
